I have always used the paper method for setting the first layer height as well. If the first layer is too flat for your liking, you can simply adjust the Z axis endstop screw by a quarter turn or so until it's how you like it. I like having my first layer squished really, really low. And that's OK....

Can we continue this discussion for those of us learning. Do you set that thickness when the bed and nozzle are *cold* or *hot*? I've found that there are times when the control thinks it's smarter than you are and it may think that it's at the end of it's throw in all directions and as such won't move. I've been running into similar issues. It seems to be if the bottom of the print is solid and then has 'walls' (think like a vase), the layers for the solid bottom will be all screwy, but then the walls will print nice and smooth. I've noticed that when I move the bed by hand (with the power off of course), that there is binding and tightness as the bed gets closer to the motor. Farther away and it frees up nicely.
