It looks like the heat break might not be tight against the nozzle, which leaves room for filament to come out the top side. Did you build this toolhead, or did you buy it from Lulzbot?

It looks like you've got some z-binding going on. Usually leveling the x-axis will help correct this problem and we've got a guide that goes over it located here: https://ohai.lulzbot.com/project/leveling-x-axis/maintenance-repairs/ The guide shows a Taz 6, but the steps are the same for a Mini. Turning off the power is fine to do when the printer is in pretty much any state. If you see something going on that shouldn't be, the first thing we generally recommend doing is powering off the printer before there is a chance of anything bad happening.

When you installed the latest version of Cura on your computer, did you then flash the firmware to the Taz 5 with it? If you are not sure hot to flash the firmware on the machine, you will select the printers name from the upper right corner of Cura, and then select Manage Printers from the drop dow...

In the last picture you posted, it looks like your z-offset could use some adjustment, due to the waves that are showing in the print. This could also be why the prints aren't sticking to well. You will want to use the latest version of Cura when using the Micro toolhead, which is currently Cura LE 3.2.27. It does use 3mm filament. One thing to keep in mind is that we have not released the adapter yet, for it to fit on the Taz. Hopefully it should be out in about 6 weeks.

I would recommend getting a new thermistor, just to prevent problems that could arise with trying to re-attach the wires.
