Had this happen a couple times recently. The nozzle seems shift to a very different X,Y position during the print. In both cases, the plastic base remained glued to the same part of the print bed.
Any idea what’s going on here?

I have seen this caused by failing rambo boards, broken limit switch wires and loose belts or pulleys.
Turns out in this case it was the result of a wire coming into contact with the print bed during motion. My z-limit switch mount had been damaged for some time, but I didn’t realize the damage was critical.
Fortunately, this issue only affected larger prints. I was able to print out a new z-limit switch mount and as soon as I had replaced it and re-harnessed the wire bundle, the problem was resolved.
