Print goes "crazy" mid-print

I am pretty new to 3d printing, and recently my Lulzbot Mini started behaving oddly on every print.

I've done the following:
1. checked to make sure printer is level
2. changed the filament spool to a brand new one
3. updated firmware of printer

Here is what's happening to Rocktopus:

The print seems to stop adhering to the layer under it at the problem spot? The problem area looks the same across different print attempts of different files, and always seems to happen after about a cm or two in height.

I'm not sure if this is a software or hardware problem at this point.

Any help is really appreciated! Thanks!!!
