Since the slope seems to directly correlate to what’s being shown in your prints, I’m suspicious that the bed leveling is not being activated as part of your startup. Also, this:

Yep. You probe the bed, but you never enable the leveling matrix. Make this simple change to your startup GCODE.
Change this:

Is this something that should be enabled in Cura by default? I added the OctoLapse preamble to my printer settings in Cura, but it should not have overwritten any previous code.
Is there a way to enable the M420 S1 command in Cura? Or do I need to add the gcode manually in the printer settings?
Yes, it absolutely should be enabled. I’ll have to download the latest CuraLE and see if this was a problem in the latest version, of if you had some custom settings from a previous install that missed this command. I checked my older copy of 4.13.1 and the error is there.
In cura, just go to your printer settings, in startup GCODE you’ll see the appropriate spot that previously only had the G29 command.

The print is still showing drift in the z height of the nozzle.

It’s looking much more consistent now, but a little close, so you can probably increase the Z offset slightly.
The right side measurements are showing the greatest deviation between the two meshes you’ve posted, with the front right being the furthest off. Could be a fan or draft that’s slowing or preventing that part of the bed from reaching proper temp. Depending on how long it’s been heating, or temp before heating, measurements would vary based on the actual temperature at that spot. (also, this is from a grand total of two measurements, so there’s not a ton of data to make conclusions on)
This inconsistency the further it gets to the right would be one indicator of something mechanically unstable toward that side between measurements. Could be something like a fan or draft on that side is causing it to be a different temperature, and thus a slightly different shape, between when it’s initially heated for the probe and when the printing begins.
The startup GCODE waits for the bed to reach 60c, but that’s at one spot in the bed, it may need to take a little longer to even out across the bed.
Manually set the bed temp to 60 and let it settle for about 5-10 minutes before starting a print.
I let the print bed warm up to 60 deg C and used my infrared thermometer to check the edges. There’s a slight different: 55 on the left, 57 on the right.
I waited 10 minutes and checked again. The sides were then more even at about 55.7 deg C.
After printing, it’s starting to look more consistent (not perfect, but better).

Thoughts on where to go from here? Should I do a bed leveling on the machine so that it stores the mesh then get rid of the G29 and M420 S1 commands in Cura so that it doesn’t repeat the bed leveling on every print?
Does 55 deg C seem too low from that thermometer? I can try to bump the plate target temp to 65 deg C to see if that makes a difference.
That’s probably as good as you can get it given the left to right downward slope in your mesh.
It’s still too few measurements to really analyze, but the consistency between the 2nd and 3rd measurements is very good, which makes me think that the initial matrix you posted reflected an bed with temperature still stabilizing.
The last picture of your first layer I would say is still a bit close to the bed. Maybe take it up another .025 to .05mm
But where there’s a very consistent height difference between left and right sides, X axis twist is my most likely explanation. The slope is easily compensated for, but if there’s a measured slope due to the twist that doesn’t exist, or more slope than measured, it’s not going to help.
I built Marlin with the X twist enabled. I don’t have a sidekick to test with, but the only changes were to add the Z probe functions for twist. Give it a try - the twist will be in the advanced settings → probe offsets → X-Twist Wizard (should be on the bottom, so you’ll have to scroll down a screen to see it).

It looks like there is still some slight adhesion issues. Once my textured bed arrives, I’ll try again and see if that helps.
My lessons learned:
- Leveling mesh is disabled by default in Cura LE for some reason. I’ve filed an issue here: Mesh not saved after G29 (auto leveling) operation (#245) · Issues · LulzBot 3D / Cura LE / Cura LE · GitLab. In the meantime, the
M420 S1 command should be added to the startup gcode in Cura just after the G29 command.
- My Z-probe offset wizard is off by about 0.05 mm. Each time I run the wizard, I should just raise the nozzle by about 0.05 mm via LCD interface.
- Let the bed warm up for 5-10 min before doing an auto-leveling probe (i.e. before every print)
- PEI can be finnicky about adhesion, depending on make/model and smooth/textured. Some things that have helped: always wait for the part to cool before pulling it off the plate, wipe the bed with isopropyl alcohol between prints, light sanding (1000+ grit sandpaper) and a soap/water washing of the plate can help (do this only rarely).
- X-axis twist can be a potential problem, but that does not seem to be happening here (please let me know if I’m wrong here). If the x-axis still seems to be off, @Wrathernaut 's custom Marlin firmware could help.
